Nursing Case Management

In addition to the home care support services, our nurses provide routine assessment of client needs and then facilitate appropriate care plan for implementation and evaluation, ensuring clients and their families have access to the services they need.

The increased complexity of care needs of an acutely or chronically ill client, mandates need for prompt and seamless access to resources that will prevent rapid deterioration and afford recovery to pre-illness state.

Our nurses bring their strong case management experiences and knowledge of community resources, to facilitate collaborative and integrative approach to quality services for our clients and their families.

Following acute or chronic changes in the functional and cognitive status of a client, experts report that early functional goal setting; plans to regain independence and referral to appropriate services are some of the primary focus of the clients and their families.
